If you have been diagnosed with COVID-19 and you have not been released from quarantine – DO NOT APPEAR.
If you have been in direct contact with someone who has been diagnosed or traveled to a high-risk area within the last 14 days – DO NOT APPEAR.
If you are experiencing symptoms such as fever, cough, shortness of breath, or any other respiratory illness symptoms – DO NOT APPEAR.
If you are a high-risk individual as defined by CDC – DO NOT APPEAR.
