• Co-Petitioner – co-signer of Petition
• Respondent - File Response to Petition, pay fee; or Waiver of Service (if not served).
  • Divorce or Separation - JDF 1015 Response
  • Allocation of Parental Responsibility - JDF 1420 Response
• Read your orders (Case Management Order).
• Prepare rough drafts (do not file yet) of:
  • JDF 1111sc - Sworn Financial Statement.
    • JDF 1111ss - Supporting Schedules (if needed).
  • JDF 1104 - Certificate of Compliance.

  • How to request Service by Publication 
  • Form for Request for Service by Publication/Consolidation notice 
  • Required Order

At that Meeting

• Clarify deadlines.
• Review draft forms and process with all forms.
• Ask non-legal questions.

Deadline Reminder for all DR cases

Within 42 days of filing the Petition (or proof of service date; or waiver of service), you must file:

• JDF 1111sc - Sworn Financial Statement.
  • JDF 1111ss - Supporting Schedules (if needed).

Within 49 days of filing the Petition (or proof of service date; or waiver of service), parents with children under 19 years of age:

• Attend Parenting Class (this may be on line, virtual, or in person)
  • Class will provide you the certificate to file. The provider does not file this certificate.
